If your work involves handling food, use the application form and contact the health department in your parish for processing and training arrangements.
Prepare your documents
The SERHA source page lists two recent passport photographs, valid identification and a Taxpayer Registration Number (TRN). Students should also bring their student identification.
Submit through your parish health department
Take the completed form and supporting documents to your parish health department. Confirm the current fee, training schedule and renewal requirements with the department before attending.
